Diet in the primary prevention of cardiovascular diseases: PREDIMED STUDIES

The Mediterranean Diet (MedDiet) is considered a model of healthy eating and has been associated to a decreased risk of several diseases and conditions using prospective cohort studies. In addition, MedDiet has demonstrated beneficial effects in several cardiovascular risk factors using randomized clinical trials. The PREDIMED trial has demonstrated that, compared to advice to adhere to a low-fat diet, a long-term intervention with MedDiet supplemented either with virgin olive oil or nuts improve different classical and emergent cardiovascular risk factors, reduces the risk of diabetes, the incidence cardiovascular disease and mortality and other chronic conditions. The PREDIMED-Plus study is an ongoing parallel group, multi-centre, randomised, primary prevention trial on men and women (55-75 years), with overweight/obesity and metabolic syndrome. The objective of PREDIMED-Plus is to compare the cardiovascular effect of two interventions: a) intensive weight-loss intervention on lifestyle program with hypocaloric MedDiet, physical activity and behavioural support, b) recommendations on MedDiet diet following the usual care of medical physicians. The principal end-points are: 1) a composite of cardiovascular events and mortality, and 2) weight loss and weight maintenance at long term. Other secondary end-points include the incidence of several chronic conditions or diseases associated to obesity and metabolic syndrome. The rationale for the new PREDIMED-Plus trial was to go a step further than the PREDIMED trial and to find out whether long term intentional weight reduction through promotion of physical activity and an energy-restricted MedDiet results in a reduction of CVD events. The main hypothesis that is being tested in this trial is that addressing energy reduction with a high-quality dietary pattern such as the MedDiet plus increased physical activity during a 6-year period, an even stronger reduction in incident CVD will be observed by comparison with that attained with a MedDiet alone in PREDIMED. We can present evidence demonstrating that the Mediterranean diet is useful in the prevention of cardiovascular disease and other health outcomes, as well as the potential mechanisms implicated.

Jordi Salas-Salvadó is currently Distinguished Professor of Nutrition and ICREA-Academia Researcher at Rovira i Virgili University (URV). He is also Director of the Food, Nutrition and Mental Health Research group recognised by the Government of Catalonia, Principal Investigator of the CIBER Network Physiopathology of Obesity and Nutrition (CIBERobn) of the Carlos III Health Institute, and coordinator of its nutrition programme which includes 23 Spanish state research groups. Professor Salas-Salvadó is also the Director of the Catalan Nutrition Centre of the Institute of Catalan Studies (CCNIEC), a networked centre that brings together 27 research groups from Catalonia and Catalan-speaking areas. He is also Chairman of the Professorship (Cátedra) Tree Nut World Forum for Nutrition Research and Dissemination of the Rovira i Virgili University, and member of the Group of Experts of the Public Health Agency of Catalonia. He became Medical Doctor at Universitat Autònoma de Barcelona in 1982, continued with several diploma in Nancy, Barcelona and Paris in Nutrition, Biology and Biotechnology and completed his PhD in 1985 at the Universitat de Barcelona. He holds numerous awards and is active in several national and international societies and boards.
